Please bring your insurance information to each appointment so we can review it and/or make any needed updates into our system. This will allow us to appropriately bill your insurance plan for services provided and help alleviate financial responsibilities to you.
If you are unable to provide current insurance information, any services provided will be your responsibility and you will be asked to pay for your visit and/or procedures done at the time of service. We accept cash, checks, and Visa, MasterCard, and American Express credit and debit cards.
